Biostimulators are a category of injectable treatments that go beyond surface-level correction. Rather than simply filling wrinkles or adding temporary volume, biostimulators activate your body’s own collagen production machinery — rebuilding the structural foundation of your skin from within for results that are both progressive and long-lasting.
At WunderLife, Dr. med. Désirée Grawunder offers two clinically established biostimulators, each with distinct properties:
Composed of calcium hydroxyapatite (CaHA) microspheres suspended in a gel carrier. CaHA is a mineral compound that occurs naturally in human bone, making it highly biocompatible. Radiesse provides immediate structural volume from the gel carrier while the CaHA microspheres stimulate new collagen and elastin production over the following months. It can also be used in a hyperdiluted form for broad skin quality improvement without added volume.
Contains poly-L-lactic acid (PLLA), a biodegradable synthetic polymer that has been used safely in medical devices for over 20 years. Sculptra is a pure biostimulator — it does not provide instant volume. Instead, the PLLA microparticles settle in the tissue and gradually stimulate the body’s own collagen synthesis over several months, producing natural-looking volume restoration and improved skin quality that can last up to two years.
THE PROCESS
When injected into the deeper dermal or subdermal layers, the gel carrier provides immediate lift and structural support. Over the following weeks and months, the CaHA microspheres act as a scaffold that stimulates fibroblasts to produce new collagen Type I, Type III, and elastin. The gel is gradually absorbed, and the newly formed collagen takes over the structural role. Results are visible from day one and continue to improve for two to three months.
In its hyperdiluted form (mixed with saline and lidocaine), Radiesse can be spread across broader areas such as the neck, décolletage, and hands for skin tightening and quality improvement without volumising effect.
PLLA microparticles are injected into the deep dermis, where they trigger a controlled tissue response. The body’s immune system recognises the particles and activates fibroblasts to surround them, producing new collagen in the process. The PLLA is gradually broken down and safely absorbed by the body while the new collagen remains, providing lasting structural support.
Sculptra results develop progressively over four to six months. It is typically administered over two to three sessions spaced four to six weeks apart, with optimal results visible at the four-to-six-month mark. Results can persist for 18 to 24 months.
Dr. Grawunder selects the appropriate biostimulator based on your PRIME Profile assessment. Radiesse is favoured when immediate correction is needed alongside long-term collagen stimulation. Sculptra is preferred for comprehensive, gradual rejuvenation where patience yields the most natural outcome. In some cases, both may be used strategically in combination.
IDEAL CANDIDATES

Biostimulators are not recommended for patients with active skin infections, autoimmune connective tissue disorders, a history of hypertrophic scarring or keloids, or those who are pregnant or breastfeeding. Suitability is assessed during your PRIME Profile consultation.
YOUR EXPERIENCE
The treatment area is cleansed and marked. A topical anaesthetic or local anaesthetic (lidocaine) is used for comfort. The biostimulator is injected using a needle or cannula into the targeted tissue planes. Dr. Grawunder’s injection technique is precise and methodical, addressing specific anatomical areas identified in your PRIME Profile assessment. Sessions typically take 30–60 minutes.
Mild swelling, tenderness, and occasional bruising at injection sites are expected and typically resolve within one to three days. With Radiesse, you will see immediate structural improvement from the gel carrier. With Sculptra, there is an initial mild volumising effect that subsides as the carrier fluid is absorbed — the true results emerge gradually over months.
For Sculptra specifically, gentle massage of the treated area is recommended (five minutes, five times daily, for five days) to ensure even distribution of the product. For Radiesse, minimal post-care is required beyond avoiding intense exercise and excessive heat for 24–48 hours. Avoid blood-thinning medications and supplements for one week before and after treatment.
Radiesse: Immediate correction visible from day one. Collagen stimulation continues over two to three months, with optimal results at three months. Duration: 12–18 months.
Sculptra: Subtle improvements begin at four to six weeks. Full results develop over four to six months across two to three treatment sessions. Duration: 18–24 months.

SAFETY & SCIENCE
Both Radiesse and Sculptra are FDA-approved, CE-marked, and backed by extensive clinical research spanning decades of use in aesthetic and reconstructive medicine.
Calcium hydroxyapatite is a naturally occurring mineral found in human bone, making Radiesse highly biocompatible with an extremely low risk of allergic reaction. No skin testing is required before treatment. Published research demonstrates that Radiesse stimulates production of collagen Types I and III, elastin, and new vasculature, leading to genuine tissue regeneration rather than simple volumisation. The product is FDA-approved for correction of moderate to severe facial wrinkles and folds, as well as hand rejuvenation.
Poly-L-lactic acid has a well-established safety record from over 20 years of use in medical devices including dissolvable sutures and surgical implants. When injected, PLLA triggers a controlled inflammatory response that stimulates collagen synthesis. Clinical studies confirm that Sculptra produces measurable increases in dermal thickness and collagen density that persist long after the product itself has been absorbed. It is FDA-approved for correction of shallow to deep nasolabial fold contour deficiencies and other facial wrinkles.
Common side effects for both treatments include temporary swelling, tenderness, redness, and bruising at injection sites. Rare complications such as nodule formation can be minimised through proper injection technique and post-treatment massage (particularly for Sculptra). Neither product is reversible with hyaluronidase (unlike HA fillers), which is why precise technique and experienced medical oversight are essential.
Biostimulators are not suitable for patients with active skin infections, bleeding disorders, autoimmune connective tissue diseases, history of keloid or hypertrophic scarring, or those who are pregnant or breastfeeding.
